Does patient engagement actually improve outcomes? The global evidence

What a 19-country patient survey measures, what the portal carries into the weeks between visits, and what we will not claim from it.

5 min read

Care does not stop when the patient leaves the room. Most of it happens afterwards, at home, unsupervised — and whether the patient is engaged during that stretch is not a soft question. It decides whether the plan you wrote is the plan that runs.

This article is about the weeks between visits: what can be measured about them, what a portal actually carries into them, and which of those things a practice pays for.

What one survey actually measured

Engagement is easy to assert and hard to evidence, so it is worth being precise about the one source we attach to it. It is a survey of patient-reported experience and outcomes, not a trial of a portal, and it does not claim that software causes better results.

The evidence

The OECD PaRIS survey, covering more than 107,000 patients across 19 countries, measures how people experience care and how that experience tracks with the outcomes they report — the case for treating the period between visits as measurable rather than assumed. It is a survey of patients, not an evaluation of any product. Source: OECD, “Does Healthcare Deliver? Results from the Patient-Reported Indicator Surveys (PaRIS)”, 2025.

The stretch between visits

A fifteen-minute consultation sits inside twelve weeks of ordinary life. In that gap the patient forgets the dose, reads a result they do not understand, gets better and stops early, gets worse and waits, or has a question small enough to feel not worth a phone call and large enough to change the outcome.

None of those events reaches the practice under a paper system, because none of them has a channel. The consultation is the only moment the record hears from the patient, so the record is a series of snapshots with silence between them — and the practice discovers what happened in the gap at the next appointment, if the patient comes.

What a portal changes is not motivation. It is whether the gap has a surface at all.

What the portal carries

Four things, and they are on every plan because none of them is a premium idea. The patient sees their record and results. Secure chat gives the small question a place to go, trilingual so it can be asked in the language the patient thinks in — a point developed in language barriers and patient safety. Family accounts with caregiver shared access bring in the person who actually manages care at home. And vitals sync from Apple Health, Google Fit and Withings, so the between-visit period produces data instead of silence.

There is also a direction people forget, which is inward. Pre-visit intake completed by the patient auto-populates the consultation, so the engagement is not only the practice reaching out between visits — it is the visit starting from something the patient has already written rather than from a blank screen.

Above that sits the structured layer. Post-visit care plans, with reminders by SMS, WhatsApp or push, start at Pro, as do chat templates and doctor-only patient notes.

Patients are free and unlimited on every plan, which matters here more than it looks: a portal charged per patient is a portal a practice rations, and a rationed portal is not a channel. Whether the plan is followed once the channel exists is the subject of medication adherence; whether the patient turns up at all is the no-show problem.

What we will not publish

We will not tell you that a portal improves your outcomes by some percentage. The survey above measures patients, not products, and converting it into a claim about this software would be exactly the move that makes a citation worthless.

Nor will we publish an engagement rate to expect. The number that would matter is your own, and it is countable without us: take one month of consultations and count how many of those patients opened the portal at least once before the next visit. That is your baseline, and it is the only figure in this article that describes your practice.

Worth remembering

The portal, secure chat, family access and wearable sync are on every plan, and patients are free and unlimited. Structured care plans start at Pro. What software changes is whether the weeks between visits have a channel — not whether a patient is motivated.

Frequently asked

Do I get charged per patient?

No. Patients are free and unlimited on all three plans; the rate is per doctor, per month. That is deliberate, because a per-patient charge turns a safety channel into a cost centre.

What arrives from a wearable?

Vitals synced from Apple Health, Google Fit and Withings, on every plan. It is patient-reported data and it is treated as such: context for the consultation, not a measurement anyone signs.

Which plan do I need?

Essential, at 69 DT, already carries the portal, secure chat, family accounts and wearable sync. Pro, at 159 DT, adds structured post-visit care plans, chat templates and 200 SMS and WhatsApp credits a month; Premium, at 269 DT, raises that to 500. Per doctor, per month, billed annually, after a fourteen-day trial — the comparison is on the pricing page, and the portal sits beside the record on the platform page.

The visit is a small fraction of care, and for most of the rest the practice is not in the room. A portal does not make patients diligent; it gives the diligent ones somewhere to go and the struggling ones somewhere to be noticed.
