Online medical appointment booking: reducing no-shows

Forgotten appointments are costly. Online booking and reminders fix that.

5 min read

An empty slot is never recovered: the hour has gone. What software does about that is narrower than most vendors admit — the practice holds the calendar, the system catches collisions and sends the reminder, and the patient is attached to a record rather than a typed name.

What an empty slot costs

A missed appointment costs one consultation fee and cannot be made up. The only figure worth having is your own, and it takes two minutes to produce.

We will not publish a no-show percentage: every rate in circulation belongs to another practice, in another country, with other patients — the only rate that matters is the one in your own week. Take the empty slots in an ordinary week, multiply by your average consultation fee, multiply by the weeks you work in the year. That number is the loss, and it is yours.

That is only the part with a price on it. The rest needs no figure: the day that ends late because absences never fall where they suit you, the desk time spent calling people who do not pick up, the patient who would have taken that hour and was never asked.

A calendar that catches conflicts

The appointment calendar is on all three plans, with drag-and-drop scheduling and conflict detection. Two visits dropped into the same slot surface as a named state — "Conflict detected, two visits at 09:00 Wed" — rather than as a discovery in the waiting room.

Patients who arrive without an appointment go into a walk-in queue rather than being wedged into the grid by hand, and Microsoft 365 and Outlook calendar sync is on all three plans, so the personal calendar and the practice calendar do not drift apart.

One term is worth defining once, because it is routinely confused with booking. A patient connection is how a person is attached to their own record — by email, SMS, link, Patient ID or QR — not how an appointment is created. Connecting them once stops the same person existing twice under two spellings. The screens are on the platform page.

Reminders, and where they stop

Push and in-app appointment reminders are included on all three plans. SMS and WhatsApp reminders are metered credits: an add-on on Essential, 200 a month on Pro, 500 a month on Premium. The gate in full:

WhatsApp reminder creditsAdd-on200 / mo500 / mo
Post-visit care plans, by SMS, WhatsApp or pushNot includedIncludedIncluded

Credits can be topped up on any plan, so the monthly figure is a budget rather than a ceiling. The last row matters for chronic patients: a post-visit care plan carries the signal to come back in three months without anyone at the desk keeping a list by hand, and it starts at Pro.

Cadence is a judgement, not a measurement. A confirmation when the appointment is entered, then a reminder the day before, covers the case that actually produces absences — an appointment taken six weeks ahead, on an anxious day, that exists nowhere but in the patient's memory. On the third message we publish nothing — the only number that settles it is your own absence count before and after. Channel decides more than count in Tunisia, where the phone is the first computer.

The desk holds the calendar

Running the calendar does not require seeing clinical notes, and the two are separate permissions. Permissions are granular, individually toggled and individually logged.

Four sit in the interface: Manage calendar, Create invoices, View clinical notes, Message patients. A secretary can hold Manage calendar and Message patients and nothing else. Whether every read and write is then captured in a per-secretary audit trail depends on the plan: the full audit trail starts at Pro, and audit log search with an investigation interface is Premium.

Seats are counted per plan — one secretary on Essential, three on Pro, five on Premium. Where the line between desk and record falls is treated in delegating to a secretary without giving up the record.

What the software does not do

Doctopass does not let a patient book their own slot. The reminder is a message, not a button: there is nothing in it to press to confirm or cancel. And no freed slot is re-offered automatically — when a cancellation comes in, the hour goes back to the practice to fill.

What exists instead does most of the work. The appointment is entered by the practice, on a calendar that catches its own collisions; the patient is connected to the record by one of the five methods; the reminder goes out on every plan. Pre-visit intake auto-populates the consultation, and the portal runs in Arabic, French and English with family and guardian accounts. Secure chat and the portal are on all three plans, and that is where a request for a different day arrives.

We would rather write that limit down than let it be discovered after signature. It is why the gates above are printed rather than described: what a practice management system in Tunisia covers should be legible before the contract.

Worth remembering

Enter the appointment once, on a calendar the practice shares, and let the confirmation and the day-before reminder go out on their own. Hold slots back each day for the emergency that has not called yet. Doctopass sends the reminder and flags the collision; it does not book the slot.

Frequently asked

Can my patients book their own slot?

No. The calendar is staff-side: appointments are entered by the practice, by drag and drop, with conflicts flagged as they are created. What a patient has is a connection to their own record — by email, SMS, link, Patient ID or QR — and secure chat with the practice through the portal, on all three plans, where a request to move an appointment arrives. We would rather state that limit here than have it found after signature.

How do I reach a patient who installs nothing?

With SMS or WhatsApp credits: an add-on on Essential, 200 a month on Pro, 500 a month on Premium, and they can be topped up on any plan. Push and in-app reminders are included on all three plans. On what a patient must install to use the portal we take no position either way — settle that in a demo, on a real phone.

Can my secretary run the calendar without opening the record?

Yes. Grant Manage calendar and withhold View clinical notes; permissions are toggled one at a time. Whether each access is then recorded in a per-secretary audit trail depends on the plan: the full audit trail is Pro and above, and searching it through an investigation interface is Premium. Essential includes one secretary seat, Pro three, Premium five.

Are SMS reminders included in the price?

Push and in-app reminders are included on all three plans. SMS and WhatsApp are credits: 200 a month on Pro at 159 DT per doctor per month billed annually, 500 a month on Premium at 269 DT, and an add-on on Essential at 69 DT — and they can be topped up on any plan. Patients are free and unlimited everywhere, billing is per doctor rather than per patient or per note, and every rate starts with a 14-day free trial. Details on the pricing page.

An appointment that exists only in the patient's memory and in your paper book is the one that goes missing. To watch the calendar, the conflict flag and the reminders work together on a week of your own, request a 30-minute demo — bring one ordinary week of your own calendar, collisions included.
